A medication review is a patient-care service that seeks to enhance a patient’s understanding of, and improve the health outcomes of, their medication regimen.
The service is provided by a pharmacist through one-on-one, the in-person appointment during which the patient and pharmacist identify all medications that the patient is taking, discuss how the medications are best taken and, where appropriate, create a medication management plan to address any issues. At the end of the appointment, the pharmacist provides the patient with one or more documents listing their medications.
